My TOC is located here: http://www.jppinto.com/index.php/series if you highlight a link it is showing the URL as:

no matter what I change in the options it still shows
http://www.jppinto.comindex.php/ instead of http://www.jppinto.com/index.php/
If you go to a page in the series, all links work except for the title or link to the series on top... i believe that is still the %series_title_linked% variable

John I'm moving this to the bugs forum as it is definitely not something expected. I can't look at this immediately but will investigate as soon as I get a chance.
I'm on Windows Server 2008 with IIS7, PHP 5.2.9 and mySql 5.x. I can get you access to my server wp if needed. I notice a lot of plug-ins don't function well when hosted on IIS. If there is anything I can do to help let me know.
It was working FINE up until I installed the following:
1) installed a new theme (switched back to old theme and the issue persists)
2) upgraded to 2.9.1
3) upgraded organize series to latest version.
I'm not sure which of the 3 could have caused the issue. Finally I whacked the plug-in and reinstalled, same issue. Could it be something in the DB?
Any ideas on this issue?

Did you try regenning the permalinks (via the WordPress Permalinks page)? I'm not real familiar with the quirks of IIS...
How do I do that? there is just a "save changes" on that page, nothing else.

The best I can do for the moment is point you to the file where all the permalink stuff is -> series-utility.php.
Take a look at the get_series_permastruct() function (around line 400). If you type the expected permalink manually in your address bar does it direct to the correct page? If yes, then the problem will be with get_series_permastruct. If not, then there's something going on elsewhere.
I changed

to

now the URL is correct but when clicking on the series title i get a 404 error
I also notice if I put nothing inside Series Custom Base: option and do an update it always comes back with a "/" as the setting.
I change the $wp_rewrite->root to $wp_rewrite->front, and the URL looks a little better. I tired to set the Series Custom Base: as "series/" when I click save it returns "/series/" so it adds a / in the front. If it didnt add that first slash in there it would be looking good.

Got the message and responded. I see that the issue still isn't resolved though? Try changing the "Series Table of Contents URL:" to something other than series (like seriestoc). There is a slight possibility that there's a permalink naming conflict. Try it and let me know how it goes.
Same issue, the real issue when setting the Series Custom Base: option its adding a / to the beginning when I hit save. If it didnt do that things would be ok.

I completely uninstalled it and reinstalled the plugin - its working now. I had to select the option to remove everything from the DB. I am in the process or recreating all the series
Glad to hear things are working now John. Don't know why there were problems with the custom base not working as expected (possibly, I've got the code not escaping some characters properly, I'll do a check when I work on the next release) Thanks for sticking to it thought and posting your results here!

I've completely uninstall/reinstalled the plugin and still have the same issues John Pinto described above. We are also on IIS, I'm wondering if you've looked further into fixing this issue.
As of right now, I changed all %site_title_linked% tokens to the %site_title% token, just so I can leave the plugin live for our users to track their way through the series.
Suggestions?

Sorry but there's not anything new that I have come across. I'm not entirely familiar with IIS and honestly don't have the time right now to look into it. I do have a open ticket regarding this issue though and will eventually get around to it. I just can't give any timeline.
I've got the same problem. I re-installed everything but it still doesn't work. Still .comindex without the slash. And when I manually pput it in I get a 404 error: http://www.koiquest.com/index.php//series/cursus_visziekten_2010/
what to do now? I like the plug in very much, but it just has to work. HELP
